Free Athletic Physicals April 6th — Forms Must Be Completed ONLINE

The Cabot Athletic Department in conjunction with the Cabot Public Schools Athletic Training Department and staff, CPS sports medicine students, and Harding University's Physician Assistants will offer FREE Pre-Participating Exam (P.P.E)/athletic physicals to student athletes on Saturday, April 6th.

NEW THIS YEAR:  

*All paperwork is online (www.swol123.net) and must be completed online

*Parents/Guardians are encouraged to go online and complete the forms BEFORE April 6th

Step 1: Go to www.swol123.net.
Step 2: Scroll to the middle of the screen and click the Join SportsWare button on the right.
Step 3: Enter your School ID

All School IDs are CabotPanthers. And hit “Next”

Step 4: Enter your Student's First Name, Last Name, Email address, School group and click the Send button. You will have to set up a second account for any additional Students. You may use the same email address but set up a different password for each account.

Your request to join SportsWare will then be sent to the Athletic Trainer for review. This could take up to 24 hours. Once your request is accepted you will receive an email with the Subject “SportsWare request accepted”. Open the email and click on the link to setup your password.

Once you receive your email from the athletic trainer you can proceed with registration as follows. Once the email is sent you have 24 hours to set up a password before the link expires. If this happens simply email your Athletic Trainer and they can help with any problems.

The pre-participation physical exams are for current Cabot Public Schools athletes who are on a current roster and will be entering the 8th-12th grades in the 2019-2020 school year. Physicals will be given to the first 500 student athletes in attendance, on a first come, first served basis.

Who: Current CPS student athletes (Must be on a current roster & next year's 8th-12th graders)

What: Free student athlete physicals

When: Saturday, April 6, 2019 from 8:30 a.m.-12:00 p.m.

Where: Cabot Junior High North (38 Spirit Drive)

If an athlete is NOT currently on an athletic roster, they MUST go to their primary care physician to fulfill the AAA requirements of having a P.P.E. prior to participating in any athletic activity for Cabot Public Schools.
