FMLA Information

The Family Medical Leave Act (FMLA) provides job protection when an employee is out due to a serious health condition of their own, or a qualifying family member, as defined in that law. When an employee misses more than three (3) consecutive work days, we are required by federal law to advise the employee of their rights and responsibilities under the FMLA and provide them with the appropriate FMLA paperwork for completion by the physician. See links below for further information on FMLA.

The Cabot School District has contracted with Consolidated Admin Services (CAS) to handle and process all of our employee FMLA paperwork. When an employee is absent for more than three (3) consecutive days from work, or intermittently due to the same health condition, you will receive an email from CAS containing links to your FMLA paperwork. The email will be from FMLA@consolidatedadmin.com. Please watch for the email and be sure to check your Junk / Spam folder, if you haven’t received it within a week.

Things to keep in mind regarding FMLA:

  1. Please notify the Personnel office as soon as you are aware that you will need to be out on leave. We prefer that you notify us by completing this form and sending it to Human Resources. If you are unable to do that, please contact the Human Resources Department at 501.843.3363.

  2. Please make sure to contact and make an appointment with Kathy Baldwin in the Payroll office regarding your benefits, pay, paid leave days, Short Term Disability, and how the leave may affect your retirement and step pay. Part of the Short Term Disability paperwork must be completed in the Payroll office. Kathy or Jeannie Head can help you with that.   3. Please have the physician complete the FMLA paperwork and return it to either CAS or the Personnel office. The paperwork will be reviewed and you will be sent a Designation Notice, advising you whether or not your leave was approved / designated as FMLA.

  4. While you are on leave, please keep in touch with the Personnel office regarding the status of your leave, expected return date, restrictions, etc.

  6. There are additional coverages under the FMLA for any qualifying exigency arising out of the fact that the employee’s spouse, son, daughter, or parent is a covered military member on “covered active duty.”  If you need information on this, please get in touch with the Human Resources Department.
