Immunization Deficiencies - Deadline Wednesday, Oct. 1

This is a reminder that the deadline for all students to have their immunizations up-to-date is Wednesday, October 1st. If your child has a deficiency, you should have already received a notice from your child's school nurse.

After October 1st, your child may not attend school until the deficiency is corrected and an up-to-date shot record is provided to your school nurse.

The district requires one of the following immediately:

*An up-to-date vaccination record with documentation of any deficient immunizations.

*An appointment card for an upcoming vaccination.

*Proof of an Immunization Exemption application.

Immunizations can be received by your own primary care provider or from the Cabot Health Department by making an appointment (501-843-7561).

If you need assistance or would like to verify that your child does not have a deficiency, you can contact your child's school nurse.
